On November 23, 1965, Warren Holifield, husband of the plaintiff, was operating a grinding machine while an employee of Grede Foundries, Inc. The grinding wheel on the machine exploded. The grinding machine guard gave way. Parts of the wheel and guard struck Warren Holifield in the head, inflicting serious injuries. Five days later he died.
